Outreach Services

Despite the current challenges we all face with COVID-19, we have continued to think about how we can increase reach into our communities aiming to have greater impact for those that may benefit from our services.

Whilst we have provided community outreach services in Chirk and Mold for the past 18 months we have been looking at both what we can offer as well as the where we provide this.

We are currently recruiting volunteers to deliver our new information, support and signposting services, the Nightingale Community Info Point.  This aims to provide information about our services across a wider area, including, Deeside, Wrexham, Llangollen and Corwen in addition to Mold and Chirk, and ensure we can link you in with our staff back at Nightingale House.

Our experts can signpost you to other services with the help of District Nurses, Community Palliative Care and Nurse Specialists in hospital or via your GP based on your needs at the time.

Here are some of the useful topics you can seek advice and help with:

  • Breathlessness, fatigue and pain management
  • Living well and achieving goals
  • Important conversations
  • A focus on family members
  • Improving sleep

Our Outreach service allows access to the hospice but in different surroundings for people who initially may only want to know what we offer.
