I would like Nightingale House Hospice to reclaim the tax on all qualifying donations I have made, as well as any future donations, until I notify them otherwise. I understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year I may be asked to pay any difference. I understand that Nightingale House Hospice will reclaim 25p of tax on every £1 that I give.

Want to cancel this declaration
Change your name or home address
No longer pay sufficient tax on your income and/or capital gains

Higher rate taxpayers

If you pay Income Tax at the higher rate, you must include all your Gift Aid donations on your Self Assessment tax return if you want to receive the additional tax relief due to you.
