Referral to the hospice

Patients and their families are welcome to call in to the hospice during the week for information about our services without the need for a formal referral.

To receive care and support from any of our services, a medical referral is required. This can be from a GP (General Practitioner) or from the consultant in charge of your care at the hospital. They can either send a letter or complete a hospice referral form.

The referral should indicate which department or service within the hospice is being asked to assess the needs of the patient and their family.

Once the referral is received, a member of the hospice team will contact the patient. The patient will be contacted by letter if an out-patient appointment has been requested. If the referral is to a clinical service such as physiotherapy or day unit, the patient will be contacted by letter or by telephone.

The hospice will maintain contact with both the referring doctor and General Practitioner to keep them informed of the outcome of the referral.
